Some things to think about…

TAKE ADVANTAGE

  • Start an open conversation about death

  • Let your family know what’s really important to you

  • Choose a health care proxy (someone who makes healthcare decisions for you in the event that you cannot)

  • Talk to your family and friends about your wishes for end of life comfort and care

  • Help the person you care for with Parkinson's Disease, Alzheimer’s or Dementia have a say in their healthcare

  • Start a legacy project

  • Have a say in your after death plans (i.e. funeral, vigils, cremation, green burial, etc.)

  • Find an estate planner, financial advisor, insurance agent, etc.

  • Create a will or trust

  • Talk about organ donation

  2. Who is your Medical Power of Attorney?

  3. Are they aware?

  4. Do you have life insurance?

  5. Do you have a will or trust?

  6. If you have minor children, aside from the other parent, where will your children go?

  7. What would a good death experience look like for you?

  8. What is your number one fear when you think about dying?

  9. Does your family know your wishes for your end of life experience?

  10. What is more important to you? Quality of life or length of life?

  11. If medical aid in dying was an option, would you consider it?

  12. Are there people you would like to make amends with before you die?

  13. How do you want others to remember you?

  14. Who do you want at your bedside during your end of life experience?

  15. What do you want done with your body once you die? Do your loved ones know?
